Adults Social Worker- LD- Positive Pathways Team

Our client, a Greater Manchester council, is seeking an Adults Social Worker to join their LD Positive Pathways Team.

Responsibilities of this role:

  • Manage a caseload of adults with learning disabilities, promoting independence, choice, and person-centred outcomes
  • Undertake strengths-based assessments, support planning, and reviews in line with the Care Act (2014)
  • Develop and implement Positive Pathways plans, supporting individuals to achieve greater independence and access community opportunities
  • Work collaboratively with individuals, families, carers, and professionals to ensure holistic, person-centred care
  • Carry out safeguarding enquiries and risk assessments, ensuring the protection of vulnerable adults
  • Support transitions from young people’s services into adulthood, ensuring continuity of care and support
  • Coordinate and contribute to multi-agency meetings, including MDTs, reviews, and best interest meetings
  • Promote least restrictive practice, enabling individuals to live independently and safely within their communities
  • Maintain accurate and up-to-date case records in line with statutory and local authority requirements
  • Ensure compliance with relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding frameworks

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have minimum of 3 years post qualifying social work experience
